As an insurance client whose Protected Health Information (PHI) is collected or maintained by Informed Choice Insurance, you have specific legal rights regarding your medical data under the Health Insurance Portability and Accountability Act (HIPAA). These rights include:
- Right to Inspect & Copy: You have the right to inspect and obtain a copy of your health records and billing information that we maintain.
- Right to Amend: If you believe the PHI we have about you is incorrect or incomplete, you may ask us to amend the information.
- Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you for treatment, payment, or healthcare operations.
- Right to Confidential Communications: You have the right to request that we communicate with you about health matters in a certain way or at a certain location (e.g., only by mail or only at a specific phone number).
2. Permitted Uses & Disclosures
We are permitted or required by federal law to use and disclose your PHI in certain situations without your specific authorization. These include:
- For Payment & Enrollment: We may use and disclose your PHI to insurance carriers to obtain premium quotes, determine eligibility, or process enrollment applications.
- For Healthcare Operations: We may use your PHI for our internal operations, such as quality assessment, compliance auditing, and customer service evaluation.

We are required to abide by the terms of this notice currently in effect. We reserve the right to change the terms of this notice and to make the new notice provisions effective for all PHI we maintain.
4. Complaints & Contact
If you believe your privacy rights have been violated, you may file a complaint with Informed Choice Insurance or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
